Transitions from Grade 8 to 9 describes a broad array of programs, strategies and supports that help students prepare for and adjust to high school life. The exchange of information is an important aspect of successful transition planning. This exchange enables students to receive individual attention and support such as:
- a caring adult assigned to provide support during transition years
- a profile that highlights student strengths, needs and interests
- a customized timetable based on the student’s profile
